We welcome new patients and will do everything possible to answer your questions and help you feel at home. To establish a baseline of your current medical condition and history, new patients are asked to fill out our Patient Information Form. You can eliminate the need to arrive early for your first appointment by downloading, printing, and completing the form in advance. When completed, you may fax it, mail it, or bring it with you to your first appointment.
Please bring your insurance and/or Medicaid card with you to each appointment.
Please bring an up to date form of photo identification.
In addition to Medicare and Medicaid, Family Medical Specialties works with a broad range of insurance providers, including Blue Cross/Blue Shield, Coventry Health Care, Midlands Choice, United Healthcare, Aetna and others. If there’s any question, please call your plan to make sure we’re on their list of approved providers. As a service to you, we will file all your insurance claims for you. With a certified professional billing coder on staff, you can be assured they will be processed as smoothly as possible. In the event a claim is denied, we will try to rectify it with the insurance company on your behalf.
You will be asked to sign a waiver for treatments and procedures not covered by your plan. In many cases, we can work with you to arrange payment or set up a payment plan in advance. Click here to contact patient accounts.
